Roy WFFL Football Registration

Roy Recreation football Sign ups are June 1 to July 1. You can register at our office located at 2626 W. 5525 S. You will need to turn in all of the following items to be registered.

1. Registration form- Pick up at our office
2. Copy of Birth certificate – If you played last year we have it on file.
3. Proof of residency- Copy of utility bill
4. Physical – Nowcare in Roy does them for $20.00 call 801-621-3466
5. Health insurance information- Carrier and policy number if you don’t have insurance there is a waiver to fill out.
6. Fee $120.00- Includes game jersey and pants to keep and all equipment issued to player.

Office hours- Monday – Thursday 9:00 am – 5:00 pm Fridays 9:00 am – 12:00 pm

Season will begin August 2nd.
